Meet the needs of the aging population
The aging population in Minnesota presents communities with challenges and opportunities. While elders need services and support, they can also offer assistance to their peers and others.

A Living at Home/Block Nurse Program is a model of programming designed to meet the needs of elders at a low cost. With a small budget and part-time staff these programs can mobilize volunteers to serve elders. They can also educate elder consumers to better take care of themselves and their family members with chronic disease.

By building relationships and being creative, a Living at Home/Block Nurse Program, such as "Community Partners" in Two Harbors, MN can help elders live at home longer. This saves public and private long-term care dollars by preventing premature nursing home placement.
